Stop by 213 W. Main (Farmers Insurance) from 5:45 pm to 7pm on Wednesday, February 22nd or Friday, February 24th.
The following items are needed for each player:
– Completed registration form
– Copy of birth Certificate
– Player photo
– Signed parent code of conduct acceptance
– Registration fees
